In July, my cash-based clinic generated $91,000 in revenue while I worked an average of 15.5 hours per week.
Here are the numbers:
✅ 82 people booked online from Facebook/Instagram.
✅ 47 showed up for their evaluation.
✅ 15 became patients.
✅ 32% conversion rate (slightly above my normal average).
✅ 36 canceled or no-showed.
Here's the interesting part...
Almost all of that revenue came during the first two weeks of the month.
The last two weeks? Practically nothing. And I still took 2.5 days off for vacation.
This is why I tell people not to obsess over one bad week.
Business is a numbers game.
The reason this works isn't because I have some magical sales script. It's because I built a business around becoming a super specialist in chronic pain. When your clinical outcomes improve, marketing becomes much easier.
I didn't just create clinical methods I built an entire business model around them.

Differential diagnosis matters.

This patient presented with right-sided low-back burning that traveled down the right lower extremity and into the top of the foot.

While hospitalized, he was evaluated by a spine surgeon and told he did not need spine surgery. I asked whether his hip had been evaluated because severe hip pathology can sometimes contribute to referred or radiating symptoms. He reported that evaluation was unremarkable.

Then he told me something that immediately raised the stakes:

A vascular surgeon had identified an arterial occlusion in the groin, and vascular intervention was being planned.

That is not something to dismiss. Vascular pathology can be limb-threatening and requires appropriate medical management.

But from a musculoskeletal differential-diagnosis standpoint, I also wanted to assess the SI region.

I published in 2020 Root Cause SI Joint Dysfunction w/ eval procedure and protocol. I’ve been teaching this for years: SI joint dysfunction can be associated with neurological-type symptoms that may extend distally into the leg and foot.

On my examination, I identified what I classify clinically as an upslip, outflare, and sacral rotation. After addressing those findings, his reported burning symptoms resolved immediately during the session.

Does that prove the vascular occlusion is irrelevant? Absolutely not. Immediate symptom relief does not rule out vascular disease or replace vascular evaluation.

It does demonstrate why clinicians need to keep the differential broad and assess the entire patient rather than automatically assuming that burning into the foot must originate from the lumbar spine.

My next step is to use my SI protocol and address the associated muscular and movement impairments, including the right piriformis, while he continues appropriate medical follow-up for the vascular findings.

Always screen the SI. Always respect the differential.

A patient came into my clinic and told me:
“I have a spondylolisthesis.”
I asked, “What grade?”
He said, Grade 4.
In my head, I immediately thought:
I’m not treating this until this patient has appropriate medical clearance.
Then I started screening.
“Any weakness in your legs?”
Yes.
“Any numbness or neurological symptoms down your legs?”
Yes.
Now we’re talking about a high-grade spondylolisthesis WITH neurological symptoms.
He told me he didn't want surgery because he was concerned about outcomes from spine surgery.
I understand the concern.
But when spine surgeons are telling you that you have a severe structural problem that may be threatening neurological function, that recommendation needs to be taken seriously.
This is not the time for me to prove how good I am at manual therapy.
It’s not the time to “just try a few exercises.”
It’s time to refer.
He also told me another provider had previously put him on traction — and afterward, he said he could barely walk.
That’s exactly why knowing when NOT to treat someone is just as important as knowing how to treat them.
As clinicians, we have to recognize when a patient's presentation is beyond what we should be managing conservatively without further medical evaluation.
For this patient:
❌ No manipulation.
❌ No traction.
❌ No “let’s see what happens.”
❌ I’m not touching him.
Referral to a spine specialist.
One of the most important skills I've developed over my career isn't another treatment technique.
It's knowing when to say:
“This isn't appropriate for me to treat right now. You need a higher level of medical evaluation.”
Sometimes the best treatment you can provide is the referral.

Think you’re dealing with SI joint pain? Look at the hip capsule first. 🔍

Here’s a wild case study: A patient came to me dealing with severe pelvic floor pain, urinary urgency, and SI joint irritation. He was striking out with urologists and traditional pelvic floor therapy.

The clues:
❌ Walking on sand made it way worse.
❌ Standard adduction protocols caused a massive flare-up.
✅ Manual hip internal rotation gave immediate relief.

The fix: We utilized Mulligan’s mobilization with movement (SNAGs) to address limited internal hip rotation. By treating the hip capsule alongside the surrounding musculature, we finally unlocked his pain.

The key to long-term success is frequency—retraining the joint capsule requires consistent input throughout the day without overloading the tissue.

I referred this patient immediately to a spine surgeon. Here’s why:

She had injured her back a couple of weeks earlier and came in with significant pain radiating into her left lower extremity.

I asked about bowel or bladder changes and saddle anesthesia. She denied both.

But when I tested her strength, I found weakness in the L4 and L5 myotomes.

That changed the picture.

I asked whether her strength had been tested in the ER. She couldn’t remember because she had been in so much pain. It’s also possible she had 5/5 strength at the ER and developed weakness afterward as her condition evolved.

Either way, new or progressive motor weakness is something serious.

I told her she needed to be evaluated by a spine surgeon.

Part of being a good primary care provider isn’t just knowing how to treat it’s knowing when a patient needs to be referred out for further workup.
