¿Tienes UHC MedicareMax Dual Complete FL-D4 (HMO D-SNP)? A partir del 1 de enero de 2027, UnitedHealthcare lo combina con UHC Preferred Dual Complete FL-QV4 (HMO D-SNP) de Preferred Care Partners. Es un plan nuevo con una red de medicos diferente.
Si no haces nada antes del 7 de diciembre, te pasan automáticamente. Antes de esa fecha, revisa que tus doctores, tus medicinas y tus centros médicos sigan cubiertos en el plan nuevo.
Do you have UHC MedicareMax Dual Complete FL-D4 (HMO D-SNP)? Starting January 1, 2027, UnitedHealthcare is combining it into UHC Preferred Dual Complete FL-QV4 (HMO D-SNP) from Preferred Care Partners. It’s a new plan with a different doctor network.
If you do nothing by December 7, you’ll be moved automatically. Before then, check that your doctors, medicines and medical centers are still covered on the new plan.
