Create Your Avera MyChart Account Starting May 30
Our new member portal, Avera MyChart, launches in just a few days. This means you can register soon. You may have already received a text or email with an activation link and code. This is the easiest and fastest way to sign up for MyChart. And remember, you only have to register once to access both your Avera patient records and Avera Health Plans member information.

You can trust the following emails and phone numbers to sign up for MyChart and future communications:

If you receive a message to activate, just click on the activation link and follow the instructions. Please note, MyChart will have limited functionality until May 30.
