An estimated 1 in 3 employers are on spread pricing arrangements with their pharmacy benefit manager, meaning the PBM charges the plan more than it pays the pharmacy and quietly pockets the difference. Most employers never see it written out anywhere. If your self-funded plan feels like it's underperforming, this is one of the first places worth checking.
