📋 The Part A deductible is per benefit period, not per calendar year. A benefit period ends 60 days after discharge, so you can be charged more than once in the same year.

Part A skilled nursing coverage runs 100 days, but it is not all free. Days 1-20 are covered in full, and days 21-100 carry a $217/day coinsurance in 2026.

Part B IRMAA starts at $109,000 MAGI for individuals and $218,000 for joint filers. It uses income from two years prior, so 2024 income sets your 2026 premium.

Medigap and Medicare Advantage both fill the 20% cost-sharing gap Part B leaves open. Medigap pays secondary to Original Medicare and keeps you in that provider network. Medicare Advantage replaces Original Medicare with a private plan that has its own network and prior authorization rules.

One note for people still working past 65: enrolling in Part A disqualifies you from making new HSA contributions going forward.
