Nurse Case Manager

Claims Associates, Inc. is seeking a compassionate, organized, and proactive Nurse Case Manager to join our growing team in Rapid City. This position plays a key role in coordinating medical case management services, supporting injured individuals throughout the recovery process, and helping facilitate safe, timely return-to-work outcomes.

Position Summary
The Nurse Case Manager manages cases from referral through closure while serving as a liaison between claimants, employers, providers, and referral sources.
Essential Responsibilities
• Develop and implement individualized case management and rehabilitation plans
• Coordinate communication among providers, employers, claimants, and referral partners
• Review medical documentation and make clinically appropriate recommendations
• Conduct site visits and jobsite analyses to evaluate work restrictions and functional demands
• Assess home care needs, equipment, and community resources
• Prepare timely reports, case summaries, and documentation
• Support return-to-work planning and job modification discussions
• Participate in case conferences and collaborative care discussions
• Maintain accurate billing, mileage, expense tracking, and case records
• Provide exceptional service while maintaining professionalism, confidentiality, and clinical ethics
Qualifications
Required
• Nursing degree or equivalent medical education
• Active RN, LPN, or PA license in good standing preferred
• Valid Driver’s License
• Minimum of 2 years of nursing or medical field experience
• Strong communication, organization, and documentation skills
Preferred
• 3–5 years of case management experience
• Case Management certification
• Experience working with rehabilitation, occupational health, workers’ compensation, or disability management

Work Environment & Travel
This position is primarily office-based in Rapid City with periodic travel to provider offices, employer locations, and claimant appointments throughout the region.
