Sanford and Avera are both ending their contract with Humana Advantage plan for 2025. Please reach out with questions or concerns during the annual election!

Sanford Health will end participation with Humana Medicare Advantage on Dec. 31 due to ongoing challenges and concerns that negatively affect patients.
