BCBSM and BCN have made substantial changes to their Medicare Advantage plans in 2026, including reduced benefits, increased premiums, and in some cases, discontinued plans.

Health insurance premiums in Michigan are set to rise sharply in 2026. Nearly 950,000 Michiganders could see double-digit increases, with individual plans averaging a 16.8% hike and small group plans averaging 11.1%. These increases are higher than those approved for 2025.

Blue Cross Blue Shield of Michigan (the state’s largest insurer) is requesting:

18.2% increase for individual market plans
11.2%–12.4% increase for small group HMO plans

These hikes come despite the insurer dropping coverage for GLP-1 weight-loss drugs (like Wegovy and Mounjaro) for non-diabetics and reporting a $1.7 billion underwriting loss in 2024.

Priority Health seeks:

14.4% increase for individual plans
9.7% increase for small group plans

Citing rising healthcare use, prescription costs, and behavioral health services

Additionally, federal subsidies that had helped lower premiums for many will expire at the end of 2025. Without them, individuals earning over 400% of the poverty level will pay full premium prices starting in 2026.

🏛️ CMS Proposed Rule: Marketplace Integrity and Affordability

CMS has introduced a sweeping new proposed rule titled the "Marketplace Integrity and Affordability Proposed Rule", with the goal of strengthening oversight, preventing misuse, and improving plan value under the ACA. Some changes may take effect as early as late 2025, with full implementation expected in 2026.

These changes significantly impact:
• Special Enrollment Periods (SEPs)
• Documentation requirements
• Auto-renewal processes
• Agent conduct standards

🔟 Key Changes to Know:

❌ End of Year-Round Low-Income SEP
• The ongoing SEP for individuals under 150% of the Federal Poverty Level (FPL) is being eliminated.
• Enrollments outside OEP will now require a qualifying life event.

📆 Open Enrollment Ends Earlier
• OEP will now close on December 15, not January 15.
• Clients must enroll earlier to ensure coverage by January 1.

📄 SEP Documentation Required Upfront
• Verification must be submitted before coverage starts.
• Failure to provide documentation may delay or deny enrollment.

⏱️ Proof of Income: 90-Day Limit
• Applicants now have 90 days to verify income — with no extensions.
• Missed deadlines will result in loss of subsidies.

🧾 Must File Taxes or Lose APTC
• Clients must file a tax return every year to reconcile their Advanced Premium Tax Credit (APTC) — even with $0 income.
• Missing this step will disqualify them from subsidies the following year.

💸 $5 Minimum Premium for Auto-Renewals
• Fully subsidized ($0 premium) clients will be charged a $5 monthly bill if they auto-renew without updating their information.
• Intended to prevent “ghost enrollments.”

🚨 Agents Can Be Suspended Immediately
• CMS can suspend agents or brokers for suspected misconduct before a full investigation is complete.
• Stronger compliance enforcement is on the horizon.

📈 Silver Plans Will Improve
• Actuarial value (AV) of Silver plans will increase.
• Expect lower deductibles and copays for Silver plan holders starting in 2026.

🛑 DACA Recipients Will Lose ACA Eligibility
• Reverses the 2024 policy that granted ACA access to DACA recipients.
• DACA status will no longer qualify individuals for Marketplace coverage.

⚠️ Gender-Affirming Care May Be Excluded
• Starting in 2026, federal rules will no longer require Marketplace plans to cover gender-affirming care.
• Coverage will depend on state policy and insurers.

✅ What to Do Now
• Agents: Review compliance training, document thoroughly, and prepare clients for earlier deadlines.
• Consumers: Plan ahead, keep income/tax paperwork ready, and stay alert for notices from your agent.
