 Brief explanation of Covered California  Plans available for enrollment in 2017  Summary of benefits, deductibles and coinsurance  Brief explanation of subsidy and premium assistance  Required documentation necessary for enrollment  Explanation of Vision, Dental and small business plans  If anyone is interested in Medicare plans they can feel free to approach and schedule an individual appointment.

Certified Covered California Agents will be available to assist in answering any questions you may have regarding enrollment. If you choose to enroll they will assist you at NO charge. Please bring the following if you think you will be interest in enrolling:

 Birthdate  Social Security Number  Home Zip Code  Proof of current income  CAID or Driver’s License  U.S. Passport, Legal resident card, Certificate of Citizenship or naturalization documentation.

*Proof of current income of all family members applying. (A dependent’s income should only be included if their income level requires them to file a tax return.) A family is defined as the person who files taxes as head of household and all the dependents claimed on that person’s taxes. If you don’t file taxes, you can still qualify for free or low-cost insurance through Medi-Cal. **Families that include unlawfully present immigrants can apply. You can apply for your child even if you are not eligible for coverage
