These aren't your typical hospitals — they're specialized facilities designed for patients dealing with multiple serious medical conditions who need extended stays of 25 days or more. Think respiratory therapy, head trauma treatment, comprehensive rehab, and pain management all under one roof. The goal is to help patients improve and eventually make it back home.

So here's what Original Medicare costs look like for 2026 when it comes to long-term care hospital stays. They follow the same structure as regular hospital stays. For each benefit period, you're looking at a $1,736 deductible covering days 1 through 60. Days 61 through 90 come with a $434 daily coinsurance. After day 90, you can tap into your lifetime reserve days at $868 per day, with a max of 60 reserve days total. Once those are used up, all costs fall on you.

Here's a helpful tip — if you transfer from a regular hospital to a long-term care hospital within the same benefit period, you won't have to pay that deductible twice. That's a solid cost-saving detail worth knowing! 💡

Did you know you can pay your Medicare premiums through your bank's online bill pay? It's a simple and convenient option that can save you a ton of time. All you need to do is sign up for your bank's online bill pay service. You or your representative can set this up directly with your bank — Social Security and CMS are not involved in that process at all.

Before you contact your bank, make sure you have this info ready:

💰 The amount of your Medicare premium bill
🔢 Account number: Your Medicare claim number WITHOUT dashes (found on your red, white, and blue Medicare card)
📋 Biller name: CMS Medicare Insurance
📬 Biller's address: Medicare Premium Collection Center, P.O. Box 790355, St. Louis, MO 63179-0355

One more thing — some banks may charge a fee for online bill pay, but CMS does NOT charge anything for processing electronic payments. That's a win! 🎉

TURNING 65 AND THINKING ABOUT RETIRING SOON? 🎉

If you're over 65 and planning to retire in the next six months, getting your Medicare timing right is super important. You don't want gaps in coverage or unnecessary penalties sneaking up on you.

Here's the deal — if you've had employer coverage through your own job, you can delay Medicare past 65 without any penalty. But once your employment ends or that employer coverage stops, you've got an eight-month window to get enrolled. Missing that deadline means a 10% penalty for every full 12 months you delayed Part B enrollment, and that gets tacked onto your premium permanently. 😬

We always recommend contacting Social Security about three months before you retire so everything lines up smoothly for both Part A and Part B. Even if you're planning to use COBRA or retiree coverage, you should still enroll in Medicare Parts A and B to avoid future headaches. Part A is usually premium-free with 40 quarters of work history, and Part B runs $202.90 per month in 2026.

If your spouse is the one retiring and you've been on their employer plan, that same eight-month window applies to you too.

Still working past 65 with an employer of 20+ employees? You can keep just Part A while their coverage stays primary.

So here's something a lot of people don't realize — Medicare actually DOES cover certain dental services, but only when they're medically necessary and tied to specific medical conditions or treatments. Since January 1, 2023, Medicare has expanded this coverage even more, which is a pretty big deal.

Here's when Medicare will step in and cover dental services:

✅ Ridge reconstruction during surgical tumor removal
✅ Stabilization or immobilization of teeth connected to jaw fracture reduction
✅ Tooth extraction to prep for radiation treatment of neoplastic disease
✅ Dental splints used alongside medically necessary treatment of a medical condition
✅ Dental exams and treatment before cardiac valve replacement, valvuloplasty, or organ transplant procedures

The key thing to know — this coverage ONLY applies when the dental care is essential to the success of a covered medical procedure. The services also have to be performed by a Medicare-enrolled provider and meet all medical necessity requirements.
