The three-midnight rule causes more Medicare confusion than almost any other, and it can be costly.

For Medicare to cover a stay in a skilled nursing facility for rehabilitation, you must first spend three consecutive nights admitted as an inpatient in a hospital.

The complication is status. A hospital may keep you for three nights but classify the stay as observation rather than inpatient admission. Observation does not count toward the requirement, even though you're physically in a hospital bed.

Many patients discover this only when a rehab bill arrives, because their hospital stay was observation, not admission.

Before you travel internationally, understand how Medicare treats care outside the United States.

Original Medicare, Parts A and B, generally provides no coverage abroad. If you need care overseas, the cost is yours.

There are options that help. Certain Medicare Supplement plans, including Plan G and Plan N, include a foreign travel emergency benefit. It typically covers 80 percent of emergency care abroad, after a deductible, up to a lifetime maximum, during the first 60 days of each trip. Some Medicare Advantage plans also offer emergency coverage outside the country, with terms that vary.

One of the most common misunderstandings in retirement planning is that Medicare covers long-term nursing home care. It does not.

Medicare covers short-term skilled care, up to 100 days, and only after a qualifying inpatient hospital stay and only while you're actively improving.

It does not cover long-term custodial care, meaning ongoing help with daily activities such as bathing, dressing, and eating. It also excludes memory care and assisted living once medical improvement stops.

In California, nursing home care commonly exceeds ten thousand dollars a month. That cost falls to you, your savings, or eventually Medi-Cal after you've spent down.

Extra Help, also called the Low-Income Subsidy or LIS, is a federal program that reduces Medicare Part D costs for beneficiaries with limited income and resources.

For those who qualify, it can lower or eliminate the Part D premium and deductible and cap prescription copays at a few dollars.

The income and resource limits are set each year, and they're higher than many people assume. Your home and vehicle are generally excluded from the resource calculation. Because of this, many who qualify never apply, assuming they earn too much.

If a single prescription copay ever reaches several hundred dollars, the Medicare Prescription Payment Plan offers relief.

Normally you pay your Part D copay in full at the pharmacy. For some medications, that means a large amount at one time. Under this program, your Part D plan instead spreads your out-of-pocket drug costs into monthly payments across the calendar year. The total you owe stays the same, but the timing becomes manageable.

There's no interest, no fee, and no credit check. Enrollment is optional and not automatic, so you have to opt in, either before the year begins or during it.

It's especially useful for those on high-cost cancer, diabetes, or autoimmune medications.

If an IRMAA surcharge appears after your income has dropped, Form SSA-44 may help.

IRMAA is calculated from your tax return two years back. When your income has since fallen because of a qualifying life-changing event, you can file Form SSA-44 to request a reduction based on your current income.

Qualifying events include:
- Retirement or reduced work hours
- Marriage or divorce
- Death of a spouse
- Loss of pension income
- Loss of income-producing property

You submit the form to Social Security with documentation of the event and your current income. They review and adjust the surcharge if approved.

Special Needs Plans, or SNPs, are a category of Medicare Advantage designed for specific circumstances. Two types matter most.

A Dual Eligible Special Needs Plan, or D-SNP, serves beneficiaries who qualify for both Medicare and Medicaid, known as Medi-Cal in California. These plans coordinate both programs and often add benefits such as dental, vision, hearing, transportation, and over-the-counter allowances.

A Chronic Condition Special Needs Plan, or C-SNP, serves those managing conditions like diabetes, heart failure, COPD, or end-stage renal disease. Care teams and benefits are built around the condition.

If you live in California, Texas, or Florida, this is one of the most important things to understand about Medicare Advantage HMOs.

In these states, enrolling in an HMO means joining more than an insurance network. You're also assigned to a specific medical group known as an Independent Physician Association, or IPA. The IPA determines which specialists you can see, which hospitals you can use, and how quickly referrals are approved.

Because of this, two beneficiaries holding the same insurance card can have very different experiences, simply because they belong to different IPAs.

When comparing plans here, the carrier name isn't enough. You have to confirm which IPAs a plan uses in your area and whether your doctors participate in them.

Here's a detail that surprises many new enrollees. If you pair Original Medicare with a Medicare Supplement, prescription drug coverage isn't built in. You'll need a separate policy known as a standalone Part D plan.

That differs from Medicare Advantage, where drug coverage is usually included.

So the two paths look like this. Original Medicare plus a Supplement plus standalone Part D means three components to manage. A Medicare Advantage plan with drug coverage combines everything into one.

Two cautions apply. Delaying Part D when you're first eligible triggers a lifetime late enrollment penalty added to your premium. And no single Part D plan covers every medication, so the plan should match your specific prescriptions.

When you compare Medicare Advantage plans, the network type matters more than the carrier name. Most plans are either HMO or PPO.

An HMO keeps costs lower by requiring in-network providers and referrals before you see a specialist. Out-of-network care is generally not covered except in emergencies. It works well if you're satisfied with your local network and rarely travel.

A PPO offers more flexibility. You can see providers in or out of network, though out-of-network care costs more, and referrals aren't required. The trade-off is a higher monthly premium. It suits frequent travelers and snowbirds.

The right choice depends on where you live, how often you travel, and which providers you want to keep.
