Attention Coyotes:
Covered California Application Assistance Workshop

Do you have questions and need assistance filing your Covered California or Med-cal application? If so, please join us!

Location: The CSUSB Student Health

When: February 24, 2014 (1pm-4pm)
February 25, 2014 (2pm-4pm)

Students MUST bring copies of the fallowing information with them to signup:

- Proof Income for all family members; w2 forms, most recent pay stubs, 1098-t form showing their financial aid and grants awards.

- Drivers License

- Permanent Residence card or Certificate of Naturalized Citizenship

- Social Security Card and date of birth from all family members in the house hold.
