America's biggest health insurers are about to get even bigger thanks mainly to the Affordable Care Act (ACA). The ACA places tougher rules on the industry, demanding more covered services, better care and a ceiling on profits. Companies are racing to capture the more than 20 million customers who will purchase coverage through the ACA's markets. Anthem is pursuing Cigna, Aetna seems close to brokering a deal with Humana, there is talk of United Healthcare going after Aetna. Its possible the big five will merge into the big three- United Healthcare, Anthem, and Aetna.

The Wall Street Journal reports that Republican leaders are backing a plan to extend the Affordable Care Act's (ACA) tax credits for up to two years if the Supreme Court rules against the subsidies in King vs Burwell this month. The proposal would extend Federal assistance for affected people while also eliminating the law's individual and employer mandates.

Studies reveal that 70% of the estimated 14.8 million Americans who use illegal drugs are employed, and drug abuse costs employers an estimated $81 billion a year. Being under the influence on the job can lead to inconsistent work quality, poor concentration and lack of focus, lower productivity or erratic work patterns, needless risk taking, and/or disregard for the safety of self and others, leading to work related accidents and injuries. While it is a problem for large employers it is more so for small companies who often times cannot afford the cost of expensive drug testing.

Obesity is closely connected to heart disease, diabetes and other conditions that can drive up health care costs for employers not to mention the individual. Obese employees may be less capable of performing basic physical tasks that their jobs require. Employers may not realize that severe obesity can shorten someone's life and time of productivity in the workforce by as much as 14 years. Studies found that the death rates in severely obese adults were about 2 1/2 times higher than in adults in the normal weight range. This risk is comparable to smoking.

Most experts are assuming there will be some sort of delay if the court issues an adverse decision for the government in the case of King VS Burwell.That's why states aren't planning what to do when the decision is made public. Also consider that most of the state's governments are not in session now.

Final enrollment in 2015 marketplace plans across the country stands at 10.2 million, according to the Centers of Medicare and Medicaid Services (CMS). CMS reported earlier this year that 11.7 million had enrolled for 2015 coverage, but many never paid the premium after enrolling. The report also shows that 6.4 million Americans receive subsidies in the federally facilitated marketplaces and could stand to lose their subsidies if the Supreme Court rules against the government in the case of King vs Burwell.
