I have had several calls about this booklet that the Centers for Medicare Services recently mailed out to recipients. These booklets are safe to fill out and mail back. (failure to do so will NOT in any way effect your coverage).

Always look for the CMS logo before filling out or mailing back any papers, booklets, or postcards. If it doesn't say CMS- Centers for Medicare & Medicaid Services then it is NOT from Medicare. Also NEVER put your Medicare number on any correspondence unless you personally know who you are sending it to. There are a lot of fraudulent companies just trying to steal your information so be careful.
