A frequency limitation, sometimes called a type of annual limit, is a rule in health, dental, or vision insurance that restricts how often a specific service is covered. Some plans or services (like an annual wellness visit) may require that at least 365 days have passed since the previous occurrence. Others may allow one visit or instance per calendar year, even if appointments are just months apart (e.g. one in August and another in April of the following year).
