Taking care of your health can earn you rewards (https://bit.ly/4vmIuYJ) . But don’t forget one important step to CLAIM them!

After you get a covered service, you must:

- Log in to the Nations portal (https://molina.nationsbenefits.com/login ) or call 844-782-2018
- Attest (confirm) the service you completed to earn your reward
- Do this within 90 days

If you miss the 90-day window, you may not get your reward.

Quick tip: Set a reminder on your phone so you don’t forget. Take care of your health and enjoy your rewards!
