This video gives all Chartered Waters Financial's New York Life policyholders an update regarding the current climate for managing your current accounts and next steps if you wish to keep Chantal as your counsel.

To give Chantal & the Chartered Waters Financial team authorization to maintain access and consulting for business as usual (monitoring policy progress, investment allocations and recommendations, etc.) please complete FORM #21114 by the following methods:

Email: Please send an email to [email protected] with "Form21114" in the subject line. We will email you a PDF for completion to grant our team telephone authorization to keep monitoring your accounts and stay on top of maximizing your contributions towards your retirement plans.

Call: You will need to call 1(800)225-5695 and request the form pre-filled and mailed to you. From their please email your form to [email protected] and our team will complete the remaining steps.

MyNYL: Form 21114 is located in your forms library. Please download & complete. From their please email your form to [email protected] and our team will complete the remaining steps.

For all other policyholders your accounts have not been affected and will not be effected because our partnerships with all other carriers are not captive contract environments.
