2017 ACA update:

A quick note for the upcoming year for all PPO plans is that starting in January, all members will need to designate a Primary Care doctor or GP to your plan. This has always been the case for HMOs, but now the rules have changed for those members of PPO plans. If you do not designate your own doctor, your plan will assign you one. This doctor will not act as a "gatekeeper" like in HMOs, but rather be a general practitioner as your go to doctor.
