Apply for Premium Tax Credits: When you apply for coverage through the Marketplace, you'll have the opportunity to see if you qualify for premium tax credits. These credits are designed to lower your monthly premium costs so you can get a plan that you don't pay for or A plan you pay a lot less for and it is based on your household income and family size. If your income falls between 100% and 400% of the federal poverty level, you may be eligible for premium tax credits.

To obtain your Form 1095-A, 1095-B, or 1095-C for the Affordable Care Act (ACA), you need to reach out to the appropriate entity based on your insurance coverage:

Form 1095-A (Health Insurance Marketplace Statement):
If you enrolled in a health insurance plan through the Health Insurance Marketplace (also known as the Exchange), you should receive Form 1095-A from the Marketplace. You can also access this form online through your Marketplace account. If you have trouble accessing it, you can contact the Marketplace Call Center for assistance by calling 1(800-318-2596).

Form 1095-B (Health Coverage):
If you had health coverage through a provider other than the Marketplace, such as Medicaid, CHIP, or certain employer-provided plans, you should receive Form 1095-B from your insurance provider. Contact your insurance provider directly if you haven't received this form or if you need assistance accessing it.

Form 1095-C (Employer-Provided Health Insurance Offer and Coverage):
If you were offered health insurance through your employer, you should receive Form 1095-C from your employer. This form provides information about the health coverage offered by your employer, including details about the coverage and any premiums paid. If you have questions about this form, reach out to your employer's human resources department or benefits administrator.

Open enrollment for ACA is between Nov 1st and Jan 15th. During this time, anyone can sign up for plans. Outside of this period, you need a qualifying life event to get major medical coverage, which could be your income if it's between $15,000 and $20,000 in most states, or if you are going to lose coverage in 60 days, or have lost health coverage in the last 60 days.

What a lot of people do not know when signing up for a health plan is you can have the hospital you go to be in network with your plan so it will cover going to the emergency room or having a surgery done but if you doctor is at the same hospital does not mean your doctor takes it so it’s important to make sure the hospitals and doctors you are with both take your health plan
