Original Medicare works anywhere in the country, and so does a supplement, because any doctor who takes Medicare takes your supplement. But an Advantage plan is tied to a service area, and when you move out of it, the plan does not follow you. The good news is that moving opens a special enrollment window. The bad news is that the window closes, and missing it can leave you on Original Medicare with no drug coverage until January.

If you do not sign up for Part B when you are first eligible, and you do not have qualifying coverage from a job, you get ten percent added for every full year you waited. Not once. Every single month, for the rest of your life. Here is the part that catches people: COBRA does not count. A retiree plan usually does not count. Marketplace coverage does not count. Ask somebody before you delay.

This is the one that costs people the most and gets explained the least. I have had someone call me at 71 wanting to move onto a supplement and find out their own health history will not let them qualify. Nothing about that is fixable after the fact. If you or someone in your family is coming up on 65, this is the deadline to know about.

Medicare Advantage is not extra coverage bolted onto Original Medicare. It is a private company running your A and B instead of the government.

Once that lands, most of the rest of this makes sense.

It does not mean free.

You still pay when you use it: something at the primary care visit, more at the specialist, a daily amount for the first stretch of a hospital stay, a percentage on the expensive things like chemotherapy or dialysis. Those are real costs on real plans. The question was never what it costs per month.

Road one is Original Medicare plus a supplement plus a drug plan. Any provider in the country that takes Medicare; no referrals, no prior authorization.

Road two is Medicare Advantage: low or zero premium, a network, copays as you go, and a cap on what you can lose in a year.
