Every year about 20-25% of my business is life insuance. Towards the end of last year I had another client pass away with little to no coverage. This year I've made a bigger effort to discuss life coverage with people and I've posted about it more.

It's May and I've already written as much life insurance as I did all last year. It's a strange feeling. Yes, it's going to make me more commission, but it's also sobering knowing that perhaps some of my clients could have had coverage or been better coverage if I had broght it up more consistently...and now that opportunity is gone.

Thinking along the spectrum of what is a necessity vs what is just a want where is life insurance? It's not like food and shelter, you can survive without it. But it's clearly more important than a new set of ear buds or your Netflix subscription.

Life Insurance should be the foundation of any basic financial plan. If you don't survive it helps complete the plan for your family or business.

There is one state in America where Walgreens can't fill a prescription.

They have a store in Fargo. It's open. You can walk in. But it doesn't sell drugs.

North Dakota.

Since 1963 North Dakota has required every pharmacy be majority-owned by a pharmacist or group of pharmacists actively working in the store. Not investors. Not private equity. Not a PBM. Not a corporation headquartered 1,500 miles away.

The pharmacist behind the counter owns the business.

Walgreens tried to kill this law. Walmart tried. Snyder Drug sued all the way to the Supreme Court in 1973. Lost.

Walmart and Walgreens lobbied the legislature twice in the 2000s. Got voted down both times.

In 2014 they put it on a ballot. 59% of North Dakotans said keep it.

And what happened to all those pharmacies that were supposed to close without corporate chains to save them?

North Dakota has more pharmacies per capita than any of its neighbors. Lowest rate of pharmacy closures in the country between 2010 and 2021. Only 17.7% closed. Nationally it was 29.8%. For independents nationwide it was 38.9%.

CVS Caremark reimburses North Dakota independents 78% higher on brand-name drugs and 37% higher on generics than it reimburses its own CVS stores. Their own spokesperson called CVS a price-accepter. Because in North Dakota the PBM needs the independent. Not the other way around.

That is the power of pharmacist ownership. When the PBM can't steer patients to its own store it has to actually pay the pharmacist fairly. The leverage flips.

Yesterday Tennessee's FAIR Rx Act crossed over in the legislature. Huge step. But Tennessee's bill says PBMs can't own pharmacies. North Dakota's law says pharmacies must be owned by pharmacists. One removes a bad actor. The other ensures the right actor is always in charge.

Nobody has ever successfully overturned North Dakota's law. It survived the Supreme Court. It survived two legislative challenges. It survived a statewide vote. Because it's not about banning one company. It's about requiring that the people who own pharmacies are the people who practice pharmacy.

We are fighting the wrong fight. Banning PBM ownership is a start. But the model that works is pharmacist ownership. 51% minimum. Active in operations. That's the law that gave North Dakota the most pharmacies per capita and the fewest closures in the country.

Stop trying to keep the fox out of the henhouse. Give the henhouse to the chickens.

Im confident physicians would love something similar given the ACA's bogus ban on POHs. (Thanks to AHA and FAH - more lobbyists similar to PCMA or PhRMA )

I was talking with someone about life insurance the other day and they mentioned they had a policy on their parents. The way they worded it was a bit odd so I asked them to explain. It turns out they live on the other side of the country and when they croak he'll have to buy airline tickes for his whole family, rent cars, rent hotels, and they will likely stay several days to visit and help out any way they can. All that will have a decent price tag on it so he has a $12,000 policy on each parent.

In 13 years of selling insurance I've never heard or thought of getting a life insurance policy to cover travel expenses.

Now the story get's even stranger.

Later the same day I'm talking to someone else about their life insurance. I mention the story from earlier in the day and they are like, "Oh yeah, we have the same thing. It's a larger policy and it all goes to an aunt that will reimburse every one."

SMH. Never heard of anyone doing this in 13 yrs and now two in one day. Are other people doing this?

This is really amazing that it happened and show that nationwide there is a need and want to reign in healthcare costs. If it happened in deep red TN then it can happen everywhere else.

Possibly the real story behind the CVS bruhaha in Tennessee is that it is just a preamble for the same battle at the federal level. Tennessee is being used to test campaign messages and talking points. The way I look at it is...The USA is the only country in the world to have PBM (Pharmacy Benefit Managers). They add very little in value to Pharmacy Benefits. In fact prescription charges, costs, and rebates are so confusing behind the scenes it would make your head spin!

A bipartisan Tennessee bill targets the company’s control over insurers, pharmacies and drug pricing — prompting a full-scale pressure campaign built on closure threats and fear of lost access.
