Sometimes it's helpful to have someone else take a look once in a while...no one person will catch everything, every time.

A couple years ago I worked with a company that overstated their deposits over the course of 4 years to the tune of $50,000+ and their CPA never caught it.

Software, like QuickBooks, does help because it reduces a lot of user errors but no one system is perfect. It helps to have great software and users in place getting to know the ins and outs of your business on a regular basis.
