It's been a long while since we posted anything onto Facebook (as Facebook reminds me daily), so I thought we'd post a little about Medicare. 'Tis the season.

Medicare coverage has changed over the last few years. You may have noticed that some of your lab bills are covered less, and we have MACRA which is digging into the way we pay providers. And on the drug plans, those who are taking certain sleeping aids or opiod-type pharmaceuticals often revert to the GOOD RX options instead of their drug plan for rate relief. [We are really helpful in this department, I'm told. Having been doing this for 30 years now, we have smart clients who teach us and we know our stuff.]

On the positive, and new, Medicare has now added some items.

Many people don't know how much original Medicare Part B (medical insurance) now covers. These six benefits might surprise you:

1. Acupuncture: If you have chronic lower back pain lasting at least 12 weeks, you can visit an acupuncturist up to 12 times in 90 days. If you improve as a result, you could get eight more visits. This benefit is only available for patients with lower back pain, but it can be very effective for this condition.

2. Therapy: Medicare Part B covers medically necessary psychotherapy, including ongoing individual therapy, group therapy, or family counseling. The number of covered mental health visits varies depending on your plan, so reach out if you would like me to check on your benefits.

3. Opioid treatment programs: These treatment programs are covered for people diagnosed with opioid use disorder. Opioid use treatment programs are pretty comprehensive—they include substance use counseling, individual and group therapy, and medication (for emergency overdoses, reducing cravings, and helping patients get through withdrawals).

4. Ambulance transportation: If you need to be transported in an ambulance, the ride might be covered based on two criteria. If you’re being transported for medically necessary services, and if a different form of transportation would be medically dangerous, then you won’t need to worry about riding out of coverage.

5. Weight loss counseling: Medicare Part B covers obesity behavioral counseling for patients with a body mass index (BMI) of 30 or above. These therapy sessions help patients evaluate and change their diets, start and stick to exercise programs, and lose weight to improve their health.

6. Getting a second opinion before surgery: If surgery is not an emergency, Medicare Part B will help you get a second doctor’s opinion in some cases. If you aren’t sure whether you should undergo surgery, you can ask to speak to another doctor before moving forward. If the second doctor contradicts your first doctor, then Part B will even cover a third opinion to help settle the matter.

If you have a Health Savings Account or participate in a Flex Plan through your employer, know that Congress has (finally!) added women's menstrual products as allowable for pre-tax purchase.

Here is a list from one administrator we work with of items they have added (and no doctor's prescription required). Eye drops, cold and headache over-the-counter items, even sunburn relief.

If your company has 5 or fewer employees and you would like to add a flex plan, this administrator is very affordable ($500/year and that includes this account, the dependent care account, smart cards for each of your participants. It's a great employee benefit that actually has benefits for the employer in F**A and work comp savings.

Victoria, thanks for putting together HBOpenforBusiness. We just put together a message for all of our insureds to (1) let them know we are here & available to assist them in any way needed; (2) remind everyone that pretty much every insurance carrier (even Medicare now) has some sort of Tele-A-Doc feature (Blue Shield, Oscar Health), "Video Visit" (Kaiser), Live Health Online (Anthem). Many have no office visit co-pay involved at all and the doctors on call can prescribe an Rx. It's not for everyone or every situation, but we know the system is in over load right now.

We have heard so many positive stories this week --- Sarah here, her gym was waiving the monthly membership fee because they're closed. Well, the members decided to keep paying their $20 so the affected staff could get paid. How resilient we are are people and let's home this dystopia right now brings out the good in all of us.

Last, those on Covered California who may have experienced a change in their income, can go online and report this and perhaps adjust their health insurance costs. Yes, you still have to keep your insurance payments going in (they're not the gym). Generally there's a 30 day grace period, and a little longer for those on Covered California. For those why rely on the individual market to buy their health insurance, 2020 Open Enrollment begins in California October 15th for a January 1st 2020 effective date (2 weeks sooner than the federal Open Enrollment begins). Also new for 2020, Californians will enjoy a state-provided subsidy for their health insurance costs if they earn more than the federal limits allow (currently 400% of poverty level, or $49,960 for single enrollees; $103K for 2020 adjusted gross income for a family of 4. Take a look. You cannot stack the two ---- and the only way to qualify for the subsidy is to enroll into a plan at www.coveredca.com (so select plan designs).

As of January 2019, AARP/UnitedHealthcare plan dropped the popular www.SilverSneakers.com benefit in favor of their new in-house, www.MyRenewActive.com. It will be free to Medicare Advantage members, but for those who are on a Medicare Supplement plan thru AARP, there will be 50% cost-sharing and your gym membership will be limited to one gym. Silver Sneakers, a very popular benefit allows members to move around from participating gym to participating gym (and also includes Silver Sneaker’s VERY popular Flex benefit for private classes outside a gym setting with credentialed private instructors).

New for 2019: The Medicare Advantage Open Enrollment period will run from January 1st to March 31st. (formerly it ended in February). If you’re enrolled in a Medicare Advantage plan, you’ll have a one-time opportunity to:
• Switch to a different Medicare Advantage plan
• Drop your Medicare Advantage plan and return to Original Medicare A and B.
• Sign up for a stand-alone Medicare Part D Prescription Drug Plan (if you return to Original Medicare).
• During this first quarter period you can also drop your stand-alone Medicare Part D Prescription Drug Plan.

Open Enrollment is also a good time to ask yourself whether you’re truly satisfied with your medical care. Remember that even if you’re happy with your current plan, things change from year to year—star ratings count, and it’s important to take the time to compare.

We have been very busy with 2019 reports for our Medicare enrollees. People enjoy when we share our trend reports. The trend has been AWAY from the AARP drug plans and even Silver Script this year, despite the 0 deductible (vs $415) on the Silver Script product. Aetna, the lower priced Humana plan and Well Care’s $15/month product have taken a good amount of 2019 market share. With Advantage plans, Blue Shield’s 65Plus drug formulary has been well received and their out of pocket spending limit (exposure for in-network expenditures by you) has dropped to $999 for 2019.

Individual health insurance

Californians will continue to enjoy slightly more flexible Open Enrollment deadlines. Nationwide, the final date to apply for individual health plan coverage is December 15th for a January 1, 2019 start. However, Californians will have until January 15th 2019 for a February 1st, 2019 plan start.

Having the extra month of “wiggle room” can be handy, as you can imagine.

Be advised that Blue Shield has gone the way of the other carriers to limit PPO day to day coverage to California services. Emergencies will continue to be covered while traveling.
