Nếu một người đủ 65 tuổi và muốn xin Medicare, thông thường cần các điều kiện và giấy tờ như thế nào?

Điều kiện cơ bản

🔹Từ 65 tuổi trở lên

🔹Là công dân Mỹ hoặc thường trú nhân hợp pháp (Green Card) sống liên tục tại Mỹ ít nhất 5 năm.

Giấy tờ thường cần

* Số An Sinh Xã Hội (Social Security Number)
* Thẻ Social Security
* Giấy khai sinh hoặc giấy tờ chứng minh ngày sinh
* Giấy tờ chứng minh quốc tịch hoặc tình trạng thường trú (nếu sinh ngoài Mỹ)
* Thông tin địa chỉ, số điện thoại hiện tại

Nếu chưa nhận Social Security

Người đó phải tự đăng ký Medicare qua:

* Online tại SSA
* Gọi Social Security
* Hoặc đến văn phòng Social Security gần nhất

Nếu vẫn đang đi làm và có bảo hiểm từ công ty

Có thể cần thêm:

* Mẫu CMS-L564 (xác nhận bảo hiểm từ chủ lao động)
* Mẫu CMS-40B để xin Part B theo SEP

Đối với Part A miễn phí (Premium-Free Part A)

Thông thường cần:

* Đã đóng thuế Medicare đủ 40 quarters (khoảng 10 năm làm việc), hoặc
* Vợ/chồng đã đóng đủ điều kiện.
