About Community

Community Health Choice is a local, non-profit, Managed Care Organization (MCO) in 20 counties in Southeast Texas. Our Harris Service Area counties are Austin, Brazoria, Fort Bend, Galveston, Harris, Matagorda, Montgomery, Waller, and Wharton. Our Jefferson Service Area counties are Chambers, Hardin, Jasper, Jefferson, Liberty, Newton, Orange, Polk, San Jacinto, Tyler, and Walker.

Community has a network of over 10,000 doctors and 77 hospitals. We are affiliated with the Harris Health System; however, we do not receive any money from Harris Health System or from Harris County taxpayers. Community offers the following health plans:

1. State of Texas Access Reform (STAR) Managed Care Program (Medicaid): We offer Children’s Medicaid and Medicaid for pregnant women—these programs are at no cost to U.S. Children’s Health Insurance Program (CHIP), including CHIP Perinatal (CHIP-P): We offer CHIP—a health insurance plan for children under the age of 19 for families who earn too much money to qualify for Texas Medicaid programs yet cannot afford to buy private insurance. CHIP-P provides prenatal care for the unborn children of low-income women who do not qualify for Medicaid.

3. Health Insurance Marketplace: We offer five deductible plans and four copay plans to provide affordable options and a range of benefits. We also help Members find out how they may qualify for healthcare premium assistance to make their insurance more affordable.

4. Medicare Advantage HMO Dual Special Needs Plan (D-SNP): We offer a plan that offers extra benefits at no extra cost low-income individuals in our service area who qualify for Medicare and Medicaid. Our Vision, Mission, and Values

This quick dinner comes together fast and uses simple ingredients you may already have.

Simple Chicken & Veggie Stir Fry

Ingredients:

1 cup cooked chicken (sliced)
2 cups mixed frozen vegetables
2 tablespoons soy sauce (low sodium if possible)
1 tablespoon oil
1 cup cooked rice

Instructions:

Heat oil in a pan.
Add vegetables and cook for 5 minutes.
Add chicken and soy sauce.
Stir and cook until heated through.
Serve over rice.
