All dental and vision plans are similar.

Essentially the premiums pay for the maintenance work you do each year and the bonus is the benefit that it pays after.

2 cleanings with xrays and an exam typically cost about $250 per visit or $500 a year

Premiums are about $40 a month per person or $480 a year with a $50 copay.

I chose Ameritas as I have found that it covers the largest network and most dentists I have found on the plan.

Extractions and fillings are paid 65% year 1 starting day 1, 2nd year they go to 80%

Major Services include

Implants

Oral Surgery

Endodontics

Periodontics

Crowns

Bridges

Dentures

These are paid 20% year 1 starting day 1, 2nd year they go to 50%

Vision is a $13 add on

It covers your eye exam with a $10 copay and it covers up to $130 for glasses.

I use this with www.zennioptical.com

Full progressive lenses with frames cost me about $70

Total cost per person is right at $49 per month per person

This pays up to $2000 per year on dental.
