The Medicare Open Enrollment begins next month, October 15. If you are on Medicare , Remember to open your Annual Notice of Changes, pay attention to what’s happening with your drug plan, and review your options. If not, you’re likely to face bad news in 2025.

Big Changes coming for 2025: After 18 rather costly years for many, this is the last hurrah for the donut hole. The IRA or the Inflation Reduction Act has eliminated it and, in its place, introduced the $2,000 cap on Part D drug costs. Once out-of-pocket costs hit $2,000, there will be no additional cost for medications.

And bad news for just about anyone with Part D coverage: The good news sounds really good for those who are reeling from price increases now but don’t start celebrating yet. The $2,000 cap will have repercussions because of the impact on Part D drug plans.

Once hitting the cap, medications miraculously do not become free; rather, the responsible party for payment changes. The portion that drug plans must pay will increase dramatically. That probably means there will be some changes to help plans absorb those costs.
Many pundits are predicting increased plan premiums, increases in cost sharing (copayments and coinsurance), more restrictions (prior authorization and step therapy) on costly medications, and even dropping drugs from the plan’s formulary or the list of covered drugs

Medicare Season is a Month Away - Read below on some recent changes to the Federal Medicare Program.

August 16, 2022, The Inflation Reduciton Act was passed which will bring some the largest changes to Medicare I have seen since I have been in the business:

The Inflation Reduction Act provides meaningful financial relief for millions of people with Medicare by improving access to affordable treatments and strengthening the Medicare Program both now and in the long-run.

The new drug law makes improvements to Medicare that will expand benefits, lower drug costs, keep prescription drug premiums stable, and improve the strength of the Medicare program.

IRA drug Part D Improvements
People with Medicare will benefit from lower prescription drug costs and a redesigned prescription drug program.

Benefits include:

Insulin available at $35/month per covered prescription
Access to recommended adult vaccines without cost-sharing
A yearly cap ($2,000 in 2025) on out-of-pocket prescription drug costs

Expansion of the low-income subsidy program (LIS or “Extra Help”) under Medicare Part D to 150% of the federal poverty level starting in 2024

Medicare will be able to negotiate directly with drug manufacturers to lower the price of some of the costliest single-source brand-name Medicare Part B and Part D drugs. This means that people with Medicare will have increased access to innovative, life-saving treatments, and the costs will be lower for both them and Medicare.

Inflation Rebates in Medicare
The new law requires drug companies that raise their drug prices faster than the rate of inflation to pay Medicare a rebate. This will lead to a stronger Medicare for current and future enrollees and discourage unreasonable price increases by drug companies.

Changes to Medicare Part B
Changes in the Medicare Part B program will improve access to high quality, affordable biosimilars for people with Medicare as well as impose a $35/month cost-sharing cap on insulin used in durable medical equipment pumps.

The Inflation Reduction Act makes Medicare stronger for current and future enrollees. It makes health care more accessible, equitable, and affordable by lowering what Medicare spends for prescription drugs and limiting increases in prices.

This report examines how Medicare beneficiaries are responding to the COVID-19 coronavirus pandemic. The
findings are based on a nationwide survey of Medicare beneficiaries who purchased Medicare health insurance
products through eHealth. The survey was conducted on a voluntary basis between April 9 and 10, 2020, and a
total of 2,914 responses were collected. Highlights:
• More than half of Medicare beneficiaries feel that more should be done to protect those at risk: 52% of
respondents do not believe enough is being done to protect those at greater risk from coronavirus; 25% say
they know someone who has contracted the virus.
• Many Medicare beneficiaries are suffering from the economic fallout of the pandemic: 37% say they have
experienced a loss of income as a result of the coronavirus crisis.
• Many cannot afford the cost of coronavirus hospitalization: 46% say they could not afford the out-of-pocket
costs they would face if hospitalized.*
Nearly all Medicare beneficiaries are observing social distancing rules: 98% say they are socially isolating,
with 81% leaving their homes fewer than 3 times per week; 68% say they are using new technologies to stay in
touch with family and friends.
• Political differences lead to different judgements of the government’s response: 58% of likely Republican
voters say they have “high confidence” in the government’s response to the coronavirus crisis, compared to
7% of likely Democratic voters.
