Big Changes Coming to Medicare Part D in 2026

Medicare beneficiaries with Part D prescription drug coverage will see major changes in 2026, thanks to continued implementation of the Inflation Reduction Act.

Here’s what you can expect:
1. Out-of-Pocket Costs Capped at $2,100
Once you hit this cap, you’ll pay $0 for covered prescription drugs for the rest of the year—no coinsurance, no copays.

2. Medicare Will Begin Negotiating Drug Prices
Starting Jan. 1, 2026, Medicare will roll out negotiated prices for 10 high-cost drugs, including: Eliquis, Jardiance, Xarelto, Januvia, Farxiga, Entresto, Enbrel, Imbruvica, Stelara, and Fiasp/NovoLog.

3. Monthly Payment Option
Starting in 2026, if you enroll in the Prescription Payment Plan (Medicare's plan that lets you spread out drug costs over the year), you will automatically be enrolled in the program the following year.

4. Insulin and Vaccine Cost Caps Continue
The insulin cap in 2026 will be the lowest of $35, 25% of negotiated price, or 25% of maximum fair price, CDC-recommended adult vaccines (like shingles and Tdap) stay free with no copay or deductible.

5. Stricter Limits on Price Hikes
Manufacturers face penalties for raising prices faster than inflation, helping to stabilize drug costs and plan premiums.

Looking ahead:
More drugs will be added to the negotiation list each year, with up to 60 covered by 2029.

Does Medicare cover assisted living or long-term care?

Many people are surprised to learn that Medicare’s coverage is very limited.

👉 What Medicare Covers (short-term, medical):
> Skilled Nursing Facility care (up to 100 days with hospital stay & doctor’s order)
> Home health services (if homebound, ordered by a physician, and needed in addition to skilled services)
> Medical services while in assisted living (doctor visits, tests, equipment)
> Hospice care (if terminally ill)

👉 What Medicare Does NOT Cover:
> Custodial care (help with daily activities like bathing, dressing, eating, etc.)
> Room & board in assisted living or memory care
> Long-term nursing home stays for daily living needs only
> Assisted living as a skilled care setting

✅ Planning Ahead
Medicare is health insurance—not long-term care insurance. Options like private long-term care insurance, hybrid life policies, annuities, Medicaid, or personal savings can help cover costs. Planning early means more options and peace of mind.

🎂Turning 65 Soon? Here's What You Need to Know About Medicare—Especially If You're Still Working

Your 65th birthday isn’t just a milestone—it’s also when you become eligible for Medicare. Whether you’re planning to retire or continue working, understanding your Medicare options now can help you avoid costly mistakes later.

✅ Already Receiving Social Security?
You’ll be automatically enrolled in Medicare Parts A and B. Coverage begins the first day of your birthday month (or the month before if your birthday is on the 1st). No action needed—but it’s still smart to review your plan options.

✅ Not Receiving Social Security Yet?
You’ll need to enroll manually. Apply online at SSA.gov/medicare, call Social Security, or visit your local SSA office. You can start the process up to 3 months before your birthday month to ensure coverage starts on time.

✅ Still Working or on a Spouse’s Plan?

- If your employer has 20 or more employees, you may delay Part B (and its monthly premium) without penalty.

- If your employer has fewer than 20 employees, Medicare becomes the primary payer—you’ll likely need to enroll in both Part A and B to avoid coverage gaps.

🚫 Important: COBRA, retiree benefits, and Marketplace plans don’t count as employer coverage. Delaying Medicare in these cases can lead to permanent late penalties.
