A Chronic Condition Special Needs Plan (C-SNP) is a type of Medicare Advantage Plan built for that. Each plan focuses on a specific condition or group of related conditions.

Depending on the plan, a C-SNP may include:

⚪ Doctors and specialists experienced with your condition
⚪ A care coordinator to help manage your treatment
⚪ Prescription drug coverage shaped around your needs
⚪ A yearly limit on your out-of-pocket costs for covered Part A and Part B services

To qualify, you need Medicare Part A and Part B, you have to live in the plan's service area, and your condition must be confirmed by your medical records.

A Medicare program is making it easier for seniors to prevent Type 2 diabetes.

New updates to the Medicare Diabetes Prevention Program (MDPP) now provide greater access for participants to join virtually and remove the one-time enrollment limit. That means more flexibility, easier access (especially for those with mobility or transportation challenges), and the ability to rejoin the program if support is needed more than one time.

The program includes health coaching, nutrition guidance, and group support to help participants lose weight and reduce their risk of developing diabetes.

With nearly half of adults over 65 living with prediabetes, these changes could make a real difference.

Did you know Medicare handles skin cancer differently than other screenings? Skin cancer is the most common cancer in the U.S., and the risk grows with age. But unlike mammograms or colonoscopies, Medicare generally doesn't cover routine skin cancer screenings if you have no symptoms.

The good news is that Medicare DOES cover the visit (and biopsy and treatment, if applicable) if you notice a change that warrants checking, like a new growth, a mole that's changing, or a sore that won't heal. That makes early detection something you can take into your own hands. Check your skin regularly, watch for the ABCDE warning signs (asymmetry, border, color, diameter, and evolving). And for heaven's sake, don't skip sun protection this summer!
