Had an interesting conversation today with a colleague and wanted to tag the group to see if anyone has used something similar. As we are seeing more commonly, insurance companies putting a frequency limitation of ‘Ortho 1x per lifetime’ clause written into the benefits (biggest culprits, Anthem and Delta of WA, NJ, CT) This can make it very difficult for plans with high benefits and patients that need two phases to be able to collect their full LTM benefit if we bill for Phase I and don’t collect full LTM in that first phase (essentially causing them to lose their remaining benefit)
Here is the question. In a discussion today a colleague said they spoke with an insurance rep and were told that they can overcome this challenge by simply billing a WIP or Continuation of treatment when patient is ready to start Phase II. I am having a hard time wrapping my mind around the logistics of how this would work, even what codes to use.
Have any of you been able to both successfully and legally navigate a loophole in this frequency limitation over two phases of treatment?
Would love to know if I am missing something here as we are seeing this frequency limitation becoming more common.

Remote Revenue Cycle Management Specialist that focuses in Orthodontic Insurance. I have 25+ years of experience in the Orthodontic field and have specialized in insurance for the previous 15 years. I handle verifications, claim submissions, insurance contract management and specialize in A/R past due clean up for offices in need of good systems and management in that area. I have a 99%+ collection rate and often run with 0 past due accounts daily, and same day followup when an accounts come into past due status. (I prefer Ortho2 edge and OrthoSync formerly OrthoPMS, although would consider other software programs) I post electronic payments and payments made via check, follow up on insurance company non-payments, discuss insurance questions with patients/responsible party when needed, place scanned EOB’s into charts as payments are posted and am willing to discuss other insurance needs that are unique to your office. I work directly within your practice management software. I personally care for and montior your accounts with the aid of one administrative assistant. I am looking to take on an additional office at this time and have excellent references. I am HIPAA compliant, insured and trained in fraud management. I also take continuing education courses in current industry standards for orthodontic insurance billing.
**I am familar with Medicaid for the state of Colorado only.
