As far back as 2018, 20,000 doctors in nearly 30 specialties claimed that the time they spent on administration was “mind-boggling,” according to the Medscape Physician Compensation Report. 70% of respondents said they spent 10 hours or more on paperwork. A third of respondents said they spent 20 hours or more on administrative duties.

Patients typically don’t realize the challenges physicians have outside of their facetime together – there are time constraints, technological difficulties, arguments and appeals, to name just a few. Physicians’ mettle is tested every single day.

It seems that increasingly, doctors are expected to take on what some call the “BSM” – business school mindset.

The issue is, doctors attended medical school, not business school.

And yet, the way the industry is going (and where it continues to head unless there’s a significant change), the business school mindset is what is being promoted and pushed – much to the detriment of the doctor, and ultimately, the patient.

We believe that the path forward to improving the patient-doctor experience is an anathema to a system that prioritizes money over medicine, profits over patients. Care cannot be effectively delivered when cost-cutting and controlling are top priority, when the doctor is as often an administrator as they are a physician.

We need a shift, and fast, one that focuses on the doctor’s ability and commitment to their patients, one that appreciates and supports their education and experience, and one that makes all elements of seeking care – from diagnosis to treatment to recovery – seamless, uncomplicated, and transparent.

Victoria Silas, MD, recently wrote an article titled “Groomers at PetSmart have more autonomy than physicians.”

She lamented over the fact that so many doctors she knows don’t have the option of scheduling longer appointments for patients who need it (because their time slots are mandated by some other entity, or because pressure from higher up requires they see more patients in less time). She wrote over the loss of freedoms doctors are suffering, and how that is directly contributing to physician burnout.

The systemic issues, she says, must be addressed, must be fixed. It’s not enough to tell doctors that they simply “need to manage their stress better.”

The end of her article read, “Systemic problems need system solutions.”

How important is trust in a doctor-patient relationship? In what situations can mistrust happen?

It can rear its head in many ways: in incorrect paperwork and documentation, in misunderstandings of treatment plans, in financial variables. When third parties and administration burdens get between a patient and doctor, that’s when mistrust often happens.

The implications of a poor doctor-patient relationship can be severe. With mistrust, patients might hesitate to seek medical care; they may be suspicious of their doctor’s recommendations; they may become pessimistic or altogether hopeless over a positive outcome.

It’s so important to restore the importance and value of the doctor-patient relationship, because it’s in that relationship that concerns are acknowledged, hope is gained, and recovery is made possible.

Insurance companies, while they staff some doctors, are not part of a medical care team.

And yet they have the power to deny payment, which effectively denies a patient the care their doctor has thoughtfully and carefully decided they need.

When doctor-patient relationships are micromanaged, unnecessary friction is created. The cost of healthcare, already much too high, rises.

When profits are put over people, healthcare becomes dollar care.

I recently came across a health insurance site where the question “what should you do if a claim is denied?” came up as one of its
FAQs.


The response?

If a claim was denied because a service or a provider was not covered by the policy, “there is not a lot you can do to fight the denial,” they wrote. “It would more than likely be a waste of time.

“To prevent anything like that from happening again, you should always closely look over your policy.”

What a mortifying answer.

So often, doctors spend an inordinate amount of time advocating for their patients, appealing the denial, explaining in as much detail as possible their arguments against the denial. Phone calls are made. Supportive documents are sent. And time is lost.

I would be remiss if I didn’t speak to the incredible value that Nurses bring to the table, and why their partnership and collaboration is so vital to rebuilding the trust between patient and doctor.


Nurses are an essential part of our healthcare system and have been dubbed the eyes and ears of doctors. Their roles are incredibly necessary as they are an integral part of the front line, ensuring patient comfort, safety and security.

For successful patient outcomes, the participation of Nurses is crucial – the intimate nature of their jobs means that their observation and interaction with patients and doctors provides them with unique, distinctive insight. Their communication, collaboration, teamwork and patient advocacy is so necessary, so needed, in modern medicine. Without them, there would be no “care team.”

For better or for worse, patients today have so much access to medical information, simply using search engines and word of mouth.

Symptoms are googled every day. Possible diagnoses – from the simple to the silly to the tragic – come up after a few clicks. Treatments, too.

Patients of course bring these to appointments and into conversations with doctors, who often recognize the well-intentioned reasons behind patients trying to self-diagnose, but who must balance expectations of care with what their education and experience have taught them is safe, reasonable and sound.

For this reason (and many others), the doctor-patient relationship is so important.

It cannot be transactional. It cannot be a process during which the patient tells the doctor what he or she must do. In the same vein, no insurance company – who is not even present in the room during examination or diagnosis – should be telling the doctor what can or can’t be done either. When others interrupt the doctor’s process, when the doctor’s education, experience, expertise, and skill is questioned, it can have disastrous consequences.

Is it not a matter of human dignity that all should be entitled to adequate, affordable healthcare?

Vxtra Health is taking significant steps to improve access to physicians in the United States, one that is accessible wherever, whenever needed, without a nightmare of red tape, bureaucracy and interruption.

But the truth right now is vastly different from what we aim to achieve – every day, thousands of Americans are surprised and distressed by what their insurance companies refuse to cover, the delays in care because of appeals their doctors must file for on their behalf, the out-of-pocket disasters that await them after treatment has been provided.

I had read that in many circles, the year 2022 signifies hope ahead. As we begin the year, I’ve been thinking about this quote in particular, by Tene Edwards:


“You must have the courage to leave the table if respect is no longer being served.”

Service must always be at the heart of healthcare, and as such, our purpose at Vxtra Health moving forward – as it has always been – is to be in service of the employers, the employees, and the doctors, all for whom we must be a voice.

I’d like to share something I heard from a gentleman whose counsel I seek frequently, an individual who actually spent a career as a hospital CEO in the days when a hospital CEO didn’t have the “you’re employed” lever to pull, and authenticity was the key to engagement.

He told me that he believed the key to engagement with doctors and nurses was what he calls the “4 U’s”. He asserts that all parties needed to feel:

👉🏼 Unified
👉🏼 Useful
👉🏼 Understood

By focusing on these principles, then the foundation of our personal and corporate brand (which at the end of the day is one and the same) will be based on authenticity. Thus, all will truly feel like they are co-creating the future.

On average, doctors spend almost a quarter of their time dealing with insurance companies, advocating for patients whose carriers deny certain treatment coverage.

If a patient needs something that doesn’t appear on a company’s list of covered procedures, it’s (initially) denied, which often results in the doctor needing to consult with another (non-practicing) doctor on the insurance company’s payroll to appeal the decision.

The appeal requires medical records and relevant, published clinical trials are sent to the insurance company. Some of the time, such documentation results in an appeal, but it’s not typical.

What usually happens is that another appeal is required, and an outside reviewer (this time, a practicing doctor) is consulted.

If this appeal is denied, the patient may have to hire a lawyer.

As you can imagine, all of this takes time. A lot of it. And many patients don’t have the time or the money to fight this, so some ask for cheaper alternatives.

I read an article recently, written by an anonymous doctor, frustrated and burned out, distraught with the state of medicine.

He called out the industry’s miscalculation of what drives a doctor: that it’s not money, bonuses or other financial incentives, but rather respect, admiration, and trust.

He shared the disappointment and fear of watching the hemorrhaging in hospitals and clinics, the loss of valuable medical professionals, the persistent scrutiny from administrators and insurers.

He spoke about the pressures of needing to reduce costs, of reaching KPIs and metrics, of achieving expected patient satisfaction scores, of needing to put profits over people.

He wonders if we’ve crossed into system collapse.

His were passionate, sad words, a painful reflection of what so many doctors see, hear and feel every day.

It makes me even more resolved to create change, to give back to doctors what they give their patients every day: respect, trust, and hope.
