1. Certificate of Employment - Latest & Original showing at least 1year of Employment
2. Proof of Billing or Barangay Clearance with the same address as COE
3. 2 valid Government issued IDs – photocopy
4. Company ID - photocopy
5. Recent 3 Months Payslip
6. Payroll ATM Card with Account Statement issued by the Bank
7. 2 pcs 2x2 ID picture
8. Accomplished Applicatiom Form.
