๐DOCUMENTARY REQUIREMENTS

1. Application Form
2. Endorsement from the Provincial/ Municipal/ City Agriculture/ Veterinary Office
3. Business Model Canvas (BMC) and Activity and Financial Plan as prescribed by the program
4. Clear scanned copy of at least one (1) valid ID
5. Proof of Residency (Barangay Certificate or valid ID)
Proof of agriculture and fishery experience and/or related training/ education, if available
6. 2 x 2 ID Photo with white background
7. Signed Disclosure Statement
8. List of Suppliers (for Processing)
9. Blueprint/ Prototype (for Digital Agriculture)
10. Individual Affidavit of New Business
EITHER OF THE FOLLOWING:
1. Certified true copy of land title or tax declaration in the name of the applicant if lot is owned by the applicant, or;
2. Rental agreement from the lot/property owner if the applicant will pay for the use of the lot/area or;
3. Authority to Use Land.
