Important Advisory to All Concerned:

Please be informed that effective MARCH 27, 2026 (FRIDAY), Caritas Health Shield, Inc. (CHSI), Caritas Life Insurance Corp. (CLIC), Caritas Financial Plans, Inc. (CFPI), and Caritas Properties Corp. (CPC) shall adopt a four-day (4-day) "Onsite" workweek, with FRIDAYS designated as "Work-From-Home" (WFH) days.

Members may file their claims and other concerns through either of the following methods:

• Submission of Claim/Application via Email ([email protected] - for CFPI Pension plans, [email protected] - for CHSI Healthcare programs) especially for Planholders/Members outside of Metro Manila area; or

• Personal filing at the CFPI Head Office during regular business hours (Monday to Thursday , 9:00AM to 3:00PM) only.

Consistent with government policy, this measure is implemented to reduce building energy consumption and transportation fuel use in light of ongoing global conditions.

LAST DAY OF FILING OF CLAIMS FOR CFPI LIQUIDATION PROCEEDINGS

To ALL Planholders AND Corporate Creditors:

REMINDER:

Today, September 16, 2025, is the last day of filing of claims for liquidation of plans of Caritas Financial Plans, Inc. (CFPI) per directive of the Insurance Commission’s Liquidation Order. You may send your claim through email to [email protected] and attach the image of your pension plan policy agreement and 2 valid government-issued ID. You may also visit CFPI today at 2nd Floor Caritas Corporate Center, 97 E. Rodriguez Sr. Avenue, Tatalon, Quezon City. CFPI is open until 3:00 PM.

Please be reminded that claims filed starting tomorrow, September 17, 2025, SHALL BE BARRED FROM THE NORMAL LIQUIDATION PROCEEDINGS BUT INSTEAD SHALL BE REFERRED TO THE COMPANY FOR RECONSIDERATION IN ITS DISSOLUTION AND WINDING UP PROCEEDINGS.

Ngayong araw po, Septyembre 16, 2025, ang huling araw ng filing ng claims para po sa liquidation ng mga plano ng Caritas Financial Plans, Inc. (CFPI) alisunod po sa direktiba na inilabas Liquidation Order ng Insurance Commission. Maaari po ninyong ipadala sa pamamagitan ng email ang inyong claim sa [email protected] at ilagay po (isama sa email) ang larawan ng inyong pension plan policy agreement at 2 valid na ID na ini-labas ng gobyerno. Maaari din po ninyo kaming puntahan upang mag file ng claim sa 2nd Floor Caritas Corporate Center, 97 E. Rodriguez Sr. Avenue, Tatalon, Quezon City. Bukas po ang opisina ng CFPI hanggang 3:00 ng hapon.

Paalala po na ang lahat ng claims na ifa-file mula bukas, Setyembre 17, 2025, ay hindi maisasama sa normal na proseso ng liquidation PERO ito ay isasama ng kumpanya sa mga isasaalang-alang sa proseso ng pagsasara at pagbuwag ng kumpanya.
