Philippine Health Insurance Corporation, known as PhilHealth, is government-owned health insurance and controlled corporation in the Philippines. Before claiming any PhilHealth benefits, members have to provide their Member Data Record or PhilHealth ID. Usually, members don’t get the PhilHealth ID even if it’s free of charge because most transactions only need to provide their membership number for most transactions.



*For Employees and Voluntary Members



Go to the nearest PhilHealth office with your two valid IDs or a photocopy of your NSO Birth Certificate. Fill out a request form and check the PhilHealth Identification Card box. Submit your paper and wait for your ID to be issued.



*For Senior Citizens



Suppose you applied via the Office of Senior Citizen Affairs (OSCA). In that case, you need to fill out two copies of the PMRF and submit both accomplished forms to the OSCA in the city or municipality where the elder lives.



If you applied at the PhilHealth office, fill out two copies of PMRF and attach a 1×1 photo of the senior citizen requesting the ID. Present the senior citizen’s OSCA-issued ID or any valid government ID. Submit the accomplished PMRF and wait for the ID to be released.
