Yes po ang sagot!

Maliban po kung ang card nyo ay Corporate, waive na ang pre-existing illness.
Kaya po lgi nmin kyo kinukulit na kumuha na habang di pa nagkaka sakit.

Eh bakit po kanyo may 1 year Pre-existing rule sa HMO? dhil pag wala lhat ng may Pre-existing illness mag apply at malulugi ang mga HMO Companies!

Pwede po kayo sa amin kumuha ng HEALTHCARD, pero kayo po ay under the CONSTESTABILITY PERIOD for Pre-Existing Illness (you can only use it for HOSPITALIZATION after a year and for Major Operation after 2years, upon Application Approval). But for other illness can use it right away.

*while under the Contestability Period for Pre-Existing Illnes.. you may use already the Healthcare Benefits for Outpatiet Service:

OUT PATIENT PREVILEGES:
Up to 15 medical consultations per year
Up to 12 pre or post natal consultations per pregnancy
Treatment of minor illnesses or injuries
Minor surgeries (those not requiring hospital facilities)
Eye, ear, nose, and throat treatment

ANNUAL PHYSICAL EXAMINATION:
(APE) after at least 3 months from the issue date, to include:
Taking of medical history
Medical Examination
Chest Xray (posterior or anterior)
CBC (complete blood count)
Stool and urine examination
Uric acid test
FBS (fasting blood sugar)
ECG (electrocardiography)
Pap smear for 30 yrs old and above

PREVENTIVE HEALTH CARE:
Periodic monitoring health problems
Consultation on exercise, diet and other healthful habits
Counseling on family planning
Vaccination but excluding cost of drugs or vaccine
Attendance at company-sponsored health seminars

DENTAL CARE:
Up to 6 consultations per year
Semi-annual oral prophylaxis after at least 3 months from the issue date
Tooth extraction but excluding surgery
Temporary filling or recementation
Treatment of oral lesions, wounds and burns

INSURANCE BENEFITS:
Term Life Insurance
200% of No. Of units purchased or a minimum of P100,000
Accidental Death Insurance
200% of No. Of units purchased or a minimum of P100,000
Waiver of Installments due to Disability

MEMORIAL SERVICE BENEFITS:
In the event of your untimely demise, your beneficiary shall be entitled to secure the services of CareHealth Plus in making arrangements for your dignified memorial service and for this purpose to utilize the anticipated proceeds, or a portion thereof but not less than fifty percent (50%) from the Term Life Insurance benefit.
