It’s an uncomfortable question.

But it highlights something important: your clinical record isn’t just an administrative task. It can become a crucial account of what happened, what you considered, what you discussed and why you made a particular decision.

“Documented” doesn’t necessarily mean “defensible”.

Clear, contemporaneous and meaningful records matter.

So, next time you finish a busy clinic, take a moment before moving on to the next patient.

A difficult case doesn’t always look difficult at the start.

Sometimes it’s an unexpected complication.

Sometimes it’s a patient who feels the result isn’t what they were promised.

Sometimes it’s a complaint that arrives months after you thought the case was finished.

And sometimes, it’s simply a decision you’re no longer sure you would make the same way today.

Being a surgeon doesn’t mean having every answer immediately.
