A yearly functional status assessment with your provider helps check in on everyday activities so you can stay safe and confident at home. This assessment is covered by your health plan, so take advantage and schedule with your provider today.

During your visit, you may talk about how you:
✔️Move safely around your home
✔️Dress yourself
✔️Bathe, shower, and use the restroom
✔️Prepare meals and eat

If you have questions call Member Services at the phone number on your Member ID Card. TTY users dial 711. From April 1 - September 30, we're here Monday through Friday from 8 am to 8 pm. From October 1 - March 31, we're here every day from 8 am to 8 pm.
