The math behind the squeeze, from the ADA's own data.

Since January 2021, dental practice costs have climbed sharply — staff wages, supplies, and equipment each up roughly 23%. General inflation over that period ran about 27%.

Average reimbursement across all payers rose about 19%.

That is a four-point gap against your own payroll and an eight-point gap against inflation, compounding for five years. The ADA also reports that dental premiums rose less than 1% between 2023 and 2024, which is part of why reimbursement has not moved — carriers are not under pressure to raise it.

The uncomfortable conclusion: the reimbursement rate itself is outside your control. What you can control is how much of what you have already earned actually gets collected — clean claims the first time, denials worked rather than written off, AR that does not age past the point of collection.

Not a satisfying answer, but it is the one that is actually available.

An Ohio dental practice agreed to pay $500,000 as part of the Department of Justice's 2026 National Health Care Fraud Takedown. It came down to one employee.

A dentist on staff had been excluded from Medicaid but kept treating Medicaid patients, with those services billed under the owner's provider credentials. Nobody inside the practice caught it. It surfaced only when the practice was sold and the new owner found the exclusion during the transition.

The exposure had been building the entire time.

Delta Dental has notified practices that paper check processing fees begin in 2027 at roughly $15 per check.

For an office receiving one check a week, that is about $780 a year. Other carriers are moving the same direction — adding similar fees or phasing out paper entirely. Some are steering practices toward third-party payment processors that take 1.9 to 3% of every payment instead. On $20,000 a month in reimbursements, that runs $4,500 to $7,200 a year, usually more than the fee it was meant to avoid.

In a survey of dental billing professionals, 78% reported an increase in claim denials or payer scrutiny over the past year. In the same survey, 71% named insurance verification as their most significant challenge.

Those two findings are connected. Tighter payer review narrows the margin for a verification gap — coverage confirmed once at scheduling is coverage that may have changed by the appointment date.

Medicare Advantage plans are offered by private companies that contract with Medicare. The plan carries a familiar commercial carrier's name, but it is Medicare — and federal rules follow it.

Practices are often brought into a carrier's Medicare Advantage network through their existing contract. Some carriers send an opt-out notice by letter or addendum; unanswered, it can result in default enrollment.
