Moving or changing your primary place of residence?

Please contact me so we can review the plans in your area. I will need to move you to the new plan or you will be reverted back to Traditional Medicare and will no longer have the extra benefits.

TRICARE FOR LIFE REMINDER: Prescriptions and medical copays are paid by Tricare for Life.

When filling prescriptions off base, please show your Military ID. Do not show your Insurance card.

When seeing Medical providers, please show both your Military ID and Insurance Card. Do not pay copays. Let providers know you have Tricare for Life that pays your copays.
