It is referred to as the GUIDE model, available to individuals with Original Medicare. This model provides a 24-hour caregiver helpline, care navigation, home safety assessments, medication reviews, and limited hours of respite care. The Guiding an Improved Dementia Experience (GUIDE) Model is a voluntary, nationwide model testing the impact of providing comprehensive services and supports for people with dementia and their caregivers.
