My client, 28 years old, a teacher has been looking for a health insurance that will suit her needs. So when I presented her FWD Set for Health, she availed for it since she was satisfied with the benefits of our health insurance despite all the discouragement she hears from other people dahil pangkritikal lang kanyang insurance iba sa HMO. And luckily her application was approved with a monthly premium amounting to 2568. Patuloy na nagbayad si client hanggang sa dumating ang di inaasahang pangyayari on the 8th month of her plan.

I received a message from my client's brother that her sister was admitted to the hospital and needs to be confined in the ICU due to elevated blood pressure and low oxygen level at kailangan nilang gamitin ang health insurance ng kanyang kapatid. Since my client's brother had no idea about the benefits of our health insurance, so I explained to him all the details. Ang FWD Set for Health ay iba sa HMO na pwedeng magamit anytime and in any cases if hospitalized. Kailangang ito ay nasa list of covered major or minor critical illnesses and for us to file for a claim, we need to comply all the required documents that will undergo further assessment. And so we waited for the doctor's diagnosis. Sad to note, my client was diagnosed with end stage renal failure at such an early age (walang pinipiling edad ang sakit) and needs to undergo a dialysis. Since it is a covered major critical illness and for us to avail the benefit we need to file for a claim with all required documents na kailangan for assessment or evaluation.

We submitted online the digital copies of all the required documents for major critical illness benefit claims (attending physician statement form, critical illness benefit claim form signed by the APS and the claimant, medical certificate and laboratory results).

We submitted the additional requirements needed dahil may hinihingi pang hypertension questionnaire ang claims dept.

We received an update from the claims department that my client's claim was approved. My client will receive the critical illness benefit amounting to 1M plus the refund of the premiums paid for the month of February and March.

The critical illness benefit was credited to the client's nominated bank account.
Sa loob lang ng maikling panahon, FWD Life Insurance Corporation pays its promise to my beloved client kahit na 8 months lang nagbayad and the benefit still continues with premiums waived. My client is still covered for two times against major critical illness (1M each claim) until age 75 and if death happens, the beneficiaries will receive 1M.
