3.Special Power of Atty. (SPA) Indicating the name of the printer who will be processing your papers specifically ATP and PCD
- SECRETARY CERTIFICATE for Corporation
4.Xerox copy of 1 valid ID with 3 wet signatures
For old customer
1.Certificate of registration
2. Penalties if there's any
3. Previous Authority to Print
4. Special Power of Atty. (Indicating the name of the printer who will be processing your papers specifically ATP and PCD
-Secretary certificate for Corporation
5. Xerox copy of 1 valid ID and 3 wet signatures
**SPA is not needed if you will be the one to process the papers with BIR
